In consideration of the sum stated hereon, inclusive of agent's service fee, and valid only upon receipt of full payment (talent fee and service fee), I hereby sell, assign, and grant to above or those for whom they act as indicated above, the right/permission to copyright and/or use, circulate, reuse, and/or publish photographs or other likeness of me in which I may be, in whole or in part, of composite or reproductions thereof in color or otherwise, still or moving, without restriction as to changes or alterations from time to time, for commercial or print advertising purposes only through any media, trade, or any other similar lawful purpose, except television, billboards, point-of-purchase displays, phone arcades, bus shelters, poster, subway, bus displays phone arcades, national ads or other high exposure usages as these require bonuses through special negotiations. Likenessness of me may be used for a period of 12 months afterwhich reusage fees must be negotiated or rights to use my photograph terminate.
The buyout fee refers to use as purchased. Please note the blocks to the left for separate bonus negotiations.
I hereby waive my right to inspect and/or approve the finished product or the advertising copy that may be used in connection therewith.
I hereby release and discharge the above, its successors and all persons acting under its permission or authority or those for who it is acting from any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite form that may occur or be produced in the taking of said picture or in any processing thereof through completion of the finished product.
All payments are due in thirty days from date of booking. Talent will receive payment based on payment from client, unless otherwise negotiated by agency.
